JC Delinquent Charts Directions
Scenario: You are the HIM Director at a hospital recently surveyed by the Joint Commission. Your department is using hybrid records. The following areas of noncompliance were reported:
· Operative reports are delinquent
· H&Ps are delinquent
In response, the Performance Improvement Committee has established a performance indicator threshold of 98.5% for monitoring ongoing compliance with meeting the operative report and H&P JC standards (measured against the average monthly operations and average monthly discharges, respectively).
You have run a report of delinquencies for the past year and now must prepare the data for presentation to the PI committee.
Directions
1. Download the  2023 Delinquent Data Report  Download 2023 Delinquent Data Report (Excel file). You will input your answers on the spreadsheet and submit the completed Excel file.
2. Calculate the average monthly discharges for the year and enter in cell B15 (highlighted in yellow).
3. Calculate the average monthly operations for the year and enter in cell B16 (highlighted in red).
4. Calculate the allowable number of delinquent records based on a 50% requirement (hint: multiply the average monthly discharges by 50% – this is the number of records allowed to be delinquent). Enter in cell B17 (highlighted in blue).
5. Compare the figure from #4 to each month’s total delinquent records to determine which months are out of compliance. Add a column to the spreadsheet to capture this data.
6. Calculate the allowable number of delinquent H&Ps based on the requirement of 98.5% completion. Compare this figure to each month’s delinquent H&Ps to determine which months are out of compliance. Add a column to the spreadsheet to capture this data.
· Hint: This means only 1.5% of records may be delinquent. Use the average monthly discharges in your calculations.
7. Calculate the allowable number of delinquent operative reports based on the requirement of 98.5% completion. Compare this figure to each month’s delinquent op reports to determine which months are out of compliance. Add a column to the spreadsheet to capture this data.
· Hint: This means only 1.5% of records may be delinquent. Use the average monthly operations in your calculations.
8. Format the data (add title/headings, remove highlighting, change column sizes to accommodate text, text formatting, etc.). It should be professional, easy to understand, and ready for committee presentation.
9. Extra credit: Use the built-in Excel formula functions for your calculations. +2 points for each correct use of formula.

A couple additional points:
1- Please remove the red and green references from your Resume.  I  had you do this exercise earlier in the semester to help you think  through how to organize.
2- Templates are not provided.  I am challenging you all to create based on the video lecture/ directions.

 Stephanie Renee Taylor Feedback:
1- Remove High School reference.
2- Tense issues.
3- Avoid first person in resume.
4- ‘Impact’ not provided in each of your ‘Experience’ bullets.
5- What are there things in brackets throughout the ‘Experience’ section?
6- Formatting inconsistencies.
7- Noticed redundant bullets – remember, this is not a quantity exercise.  It is a quality exercise.
8-  Professional Summary needs a bit of work: what is meant by the first  bullet?  What types of roles are you seeking post graduation?  You will  want to ensure the things you are highlighting in your Resume, as part  of your summary, correspond.

Abstract
Quickly tell the reader what to anticipate in this paper. Approximately 200 words. Here also mention the theory you are addressing your topic issue and paper around. Your abstract needs to be on its own page.
Keyword: Issue Topic (i.e. Option #1 – Diversity)
Introduction
This should be around two paragraphs. In the introduction, the reader should know what issue you are addressing. You should explain why examining this issue is important. This is the “hook” – to get you reader interested. This sets up the rest of your paper.
Case Studies/Review (Body of your Paper)
Body of paper with comparisons of articles and facts around theory. Here is where you are addressing your topic in a theoretical manner. Make sure the entire paper is 12-point font, New Times Roman, double-spaced with bolded section headers. The entire paper should be around 2,500-3,000 words. Make sure if you state an idea that is not your own, that you properly cite that reference using APA intext citations (Lard, 2019). Please see APA guides for help. Cite all statements that appear factual to prevent the appearance of conjecture or opinion as they are frowned upon in a scholarly research paper.
Often, I will have about one-two paragraphs that give more background regarding the topic issue and why it is important. One suggestion is to find recent data or stats that can substantiate your claim.
In the “body” of your paper, you may use “second headings” to further explore your issue. This would be were I would introduce the theory or conceptual framework that you are using. In this paper, you are going to be the subject matter expert. Assume your reader is a layperson and does not know anything about the theory or conceptual framework. I would provide an in-depth explanation of the theory; how it has been applied; critiques and criticisms; how the theory has evolved, and how it is perceived today. Write about how this theory is appropriate when looking at your issue. How does it help address the issue? Remember that this paper is your creation. These are just my opinions but if you want to write-it up differently that is your prerogative. Some papers have compared and contrasted different theories – which was very interesting. I would caution not to use more than three theories in this paper. We are looking for depth of analysis.
Discussion
This is where you wrap up your research with the theory, pertaining to the reason you wrote this in the first place, in other words, the problem.
Conclusion
Summarize and conclude your study, wrap it up concisely.

Research Proposal
(20 points, 750 words)
Objective
Students read an article and then generate a possible response to specific points made by the
writer. The student finds research to support that response. In an essay written for the
instructor, the student explains how a given piece of research will be used in the upcoming
research-based persuasive essay. The assignment engages comprehension, analysis, and
metacognition. The relevant course objectives are: 2, 3, 8, 9, 10.
The Task
Please read the article “‘I’ll Do My Own Research’: Why Americans Don’t Trust Experts
Anymore” by Stephen Altrogge. The link is in Canvas, but if it does not work, paste this link into
your browser:
https://www.trustsignals.com/blog/why-dont-americans-trust-experts-anymore
Review the assignment sheet for the Research-based Persuasive Essay. This proposal lays the
groundwork for that paper. I am your audience for the Research Proposal. You can use the first
person. You need to identify what content from the article that you’ll be addressing, and you
need to have some idea of what you want to argue. It does not have to be absolutely for
certain, but you need some direction.
Find four sources and in a single discussion paragraph for each one, identify the source, briefly
explain its content, and discuss how and where you plan to use the data in the persuasive
essay. Do not utilize quotations.
Thesis
Tell me what shall be the focus of your persuasive essay and what you opine. Briefly explain
your rationale and describe the types of information you will need to make your case. The
thesis should occupy a single, well-developed paragraph.
Discussion
Each source (four total) should be identified by author or title and the relevant content
summarized. Explain how you plan to use the data in your essay. Do not simply state the
obvious, such as “I plan to use the article to support my claim.” Be more specific without using
quotations. Each source should be discussed in one paragraph.
Conclusion
Ask me questions and voice concerns about composing the upcoming research-based
persuasive essay. My comments shall focus on your questions. If you do not ask questions,
you’ll receive a C at 70%.
